MCV4 vaccine Upcoming 11th Graders

Attention upcoming 11th graders for the 2023-24 school year, the Georgia State Department Of Health requires upcoming 11th graders to get a booster of the meningococcal conjugate vaccine (MCV4). This will be a requirement for all 11th grade students for the next school year.

You may bring your child's updated FA3231 Immunization form to school or send to Jana Bullen, [email protected], or fax: 706-861-6645. Please don't hesitate to call if you have any questions about this state requirement.
